Before your visit
What to bring so we can make the most of your time.
A few minutes of prep means more time talking about you and less time on paperwork. Arrive about ten minutes early for your first visit, and bring these four things.

Your insurance card
Bring your current card (or a photo of it). If your coverage changed recently, let the front desk know so we can verify benefits before your visit.

A photo ID
A driver's license, state ID, or passport. We confirm identity at check-in to keep your medical record accurate and secure.

A list of medications
Everything you take — prescriptions, over-the-counter meds, vitamins, and supplements, with doses if you know them. Photos of the bottles are perfectly fine.

Your copay
If your plan has a copay, bring it for check-in. Self-pay? Our posted cash price is collected at the visit — ask us for the amount when you book.
